About the role

The Community Aged and Complex Care Nurse role aims to address the growing number of clients with complex multi-morbid health conditions and social care needs, where factors such as frailty, impaired cognition and polypharmacy add to the difficulty of planning and navigating care requirements congruent with the client’s expressed wishes.

The role will incorporate the application of knowledge and skills in the areas of cognition, advance care planning and navigating the service system. Coordinating case conferencing and care planning would also be considered a key function of the role, where case management is not in place.

Skills & Experience

  • AHPRA recognised Registered Nurse qualification
  • Post graduate qualifications and/or extensive experience in aged care nursing
