Position Description
  • Part-time (56 hours per fortnight negotiable) 
  • Primarily in Lakes Entrance and offering service to outreach sites
  • 5 weeks of annual leave (pro-rata)
  • $15,900 in salary packaging available 

We have an exciting opportunity for a registered nurse to join our Clinical and Nursing services team, providing nursing services to the East Gippsland community.

The role of the primary health care nurse incorporates a range of activities which contribute to the optimal functioning of the GP Practice and supporting access to quality of primary care services to the smaller communities surrounding Lakes Entrance.

The position will work closely with other service providers including general practitioners, allied health, speciality nursing services, and other community support services to ensure a coordinated approach to client care. 

The scope of the role includes: triage, assessment and initial management of acute presentations; pathology collection; preventative health and early intervention initiatives including education, screening, coaching & ongoing monitoring of health status; bio-psychosocial assessment of clients with chronic health conditions using a client centred approach incorporating health change methodology; and documenting of shared support/ care plans in collaboration with the interdisciplinary team members including coordination of case conferences or care planning meetings as required.
